Home & Community Habilitation focuses on helping individuals learn and strengthen the skills needed to live as independently as possible. Our staff works one-on-one to support personal development, daily routines, and meaningful community involvement.
Community Integration
Building Connections • Encouraging Independence • Creating Opportunities
Community Integration support helps individuals discover and participate in activities, relationships, and experiences that are meaningful to them. Staff work alongside each individual to identify their interests, goals, and preferences while providing the support needed to participate safely and confidently in their community.
Companion Services
Tailored Assistance
Companion Services provide a supportive presence for individuals who need help staying safe and engaged in their daily routines. Companions ensure individuals are never alone when they need guidance, emotional support, or encouragement to participate in everyday activities.
Homemaker / Chore Services
Specialized Support
Homemaker / Chore Services support individuals by helping maintain a clean, organized, and safe living environment. We offer respectful, professional assistance with household tasks that may be difficult to manage alone.
Respite Care
Temporary Relief
Respite services provide short-term, out-of-home care to give primary caregivers time to rest, attend appointments, or manage personal responsibilities. While in respite care, individuals receive compassionate supervision and opportunities to enjoy structured, meaningful activities.
